Sports Physicals | School Physicals | Dearborn

For participation in school related sports activities, Michigan High School Sports Athletic Association ( MHSAA ) requires all Middle School and High School student athletes to be current on their Sports Physical Exams also known as "Sports Physicals".
As a service to our community and for your convenience,  Community Urgent Care offers Sports Physicals for only $ 25.00. No appointment is necessary.
Please be sure to print and fill out the Medical History forms to the best of your knowledge, accurately and truthfully.

Heart Conditions

If there is a family history of heart problems in close relatives like premature heart disease before age 50 or Sudden Cardiac Death before age 50, Cardiomyopathy, Prolonged Q-T interval, Marfan Syndrome or Cardiac Arrythmias (irregular heart-beat) then your student athlete MUST see your own Pediatrician or Primary Care Physician for Sports Physical clearance. 
American Heart Association  (AHA) does not recommend routine cardiac testing. If your student athlete is experiencing high blood pressure readings, unexplained fainting or near fainting spells, chest pain or shortness of breath with or without exertion, unexplained excess fatigue with exertion or has a significant heart murmur then you must see your own Pediatrician or Primary Care Physician for further plan of care.
Student athletes with prior sports activity restriction requiring cardiac evaluation will not be cleared without cardiology clearance letter. 
If your student athlete has history of heart problems and has been evaluated and cleared by a Cardiologist, be sure to bring Cardiology Clearance Letter indicating that your student athlete has been unconditionally cleared to participate in sports activities.

Sub-Optimal Vision

If your student athlete has sub-optimal vision and has been prescribed prescription spectacles, please be sure to bring them with you as these will be required for the vision test section of the Sports Physical Exam process.

A concussion is a form of a brain injury and it is extremely common. In the United States alone, there are over three million cases of concussion from sports related injuries.  
After head trauma, any athlete showing sign, symptoms or behavior changes which include and are not limited to loss of consciousness, headache, dizziness, blurred vision, confusion and loss of balance, should be immediately removed from the sports activity and should be evaluated by M.D., D.O. or a Nurse Practitioner familiar with concussions.

Hypertrophic or Dilated Cardiomyopathy is a very serious medical condition. If it is not recognized in a timely fashion, it can lead to sudden death. 
It is extremely important to fill out your Pre-Participation Health History form truthfully and accurately. Also verbally communicate to your examining physician about any symptoms you may be experiencing or if there is any family history of Sudden Death, Syncopal or fainting spells, Arrythmias or Irregular Heart Beats requiring Automatic Implantable Cardioverter Defibrillator (AICD), Marfan Syndrome, Non-Sustained Ventricular Tachycardia (V-Tach) on Holter monitor or massive cardiac wall thickness >30 mm on Echocardiography.
If you answered yes to any of the items above, you will require additional medical evaluation. you must see your Primary Care Physician or Pediatrician for further plan of care.
